What to Bring to Your appointment

• Completed Form - Your filled out "Request for Live Scan" Form (You should receive this from your employer).

• Valid Identification - Drivers License or other valid form of ID. Please click here for a complete list of acceptable identifications.

• Payment - Your form of payment: Card, Venmo, CashApp, PayPal, Cash. 

            Cost: $35+Govt. fee( to find out your government fee please visit

       with your ORI number (this will be located on your live scan form) 

• Token - IF you received a token please bring it with you. (This is if you fill out the form online. This will make the appointment quicker for you and it will notify us that you already paid. Want a Token? Fill Out the Form Here)

• Certifix Account - IF you received a certifix account number please bring it with you (You would receive this from your employer; it would alert us that the live scan is at no cost to you).
